WAIVER
Prodigy requires that all wrestlers complete the annual waiver in order to participate in Prodigy Wrestling Academy. These forms allow prodigy to keep your contact information updated each season and for parents and wrestlers to agree with Prodigy Liability Waiver. Waivers must be completed ONE TIME each season starting on August 1st and ending on July 31st of the following year.
DROP-INS AND VISITORS ARE ALSO REQUIRED TO FILL OUT THIS FORM.
PROGRAM REGISTRATION

- Registration is ONLY for when registering for a FULL PROGRAM.
- DO NOT fill out a registration form if doing a DROP-IN practice or buying a PUNCH CARD.
- ALL wrestlers MUST have a waiver filled out for this season in order to participate. See waiver section above for details.
- CLICK on the program link of the program in which you would like to register.
- PAYMENTS (Credit, Check, or Cash) are taken on the first night of participation at the front desk. There is no online payment option at this time.
- YOU DO NOT fill out a registration form for any of the programs below if doing a DROP-IN practice or buying a PUNCH CARD for that program.
